Accident Procedures

  • Be specific in describing the accident.

  • Home accidents and those not occurring under organizational jurisdiction should not be reported.

  • The iThe following procedures must be followed with every accident. Accidents are reported on the report of accident form.

Staff present should complete the report of accident form at the scene of the accident. The injured party or parties are not to make out the form in any part.

As quickly as the follow-up is complete and entered, forward the duplicate copy to [designated personnel].

The following may be important in accident reporting:

  •  Be specific in describing the accident.

  • Home accidents and those not occurring under organizational jurisdiction should not be reported.

  • The immediate supervisor or designated HR person is responsible for the follow-up of all accident cases where a loss of company time or medical care is involved

  • Designated personnel should hold the report of accident form until the follow-up has been completed and noted on the report. The person charged with supervision at the time of the accident must sign the copies.

  • The number of work days missed may be a vital statistic -- make certain that this information is entered.

The organization copy must be held until the staff member leaves the organization. At that time the HR department will review the file and keep a copy for the appropriate time.

    • Disability/allergy
    • Address
    • Phone
    • Cell phone
    • Email
    • Emergency contact
