Official Contest Rules XZY COMPANY NEW YORK CITY TRIP FOR TWO
1. To participate, complete an entry form with your name, address and phone number and drop in an official entry box at at XYZ COMPANY, 2222 ANYSTREET, ANYTOWN, USA 800XX or complete the entry form for a chance to qualify at various times on the YXZ RADIO WEBSITE, WWW.YXZ RADIO.COM [INSERT CONTEST DATES DD/MM/YY].
2. The contest will be conducted March 17 through April 28, 20XX. Deadline for entries is [INSERT DATE, 20XX] Grand prize drawing will be held on Wednesday, April 28 at XYZ COMPANY, 2222 ANYSTREET, ANYTOWN USA. 800XX AT AT 7 PM DURING THE HOLIDAY SALE.
3. Qualification for the Grand Prize drawing will be held as follows: each day from March 17- April 25 20XX, an entry will be drawn and the qualifier announced at XYZ COMPANY. A total of forty-seven (47) qualifiers will be drawn in this manner. In addition, each week from March 17 through April 28, 20XX, two entries will be drawn at YXZ RADIO studios and the qualifiers announced on the YXZ RADIO website, www.YXZ ZRADIO com and on the air at XYZ RADIO AM 250. A total of 12 persons will be qualified in this manner. Thus, a total of 59 persons will be qualified to enter into the grand prize drawing. Each qualifier and a guest will be put into a drawing AT XYZ COMPANY , and at or around 7 PM on April 28, 20XX, the grand prize winner will be drawn and announced.
4. Each qualifier will receive a gift certificate for $10 for the XYZ COMPANY. The grand prize is a trip for 2 people to New York City, NY. Prize includes (a) roundtrip airfare for two from Anytown, USA to New York City, NY; (b) 2 nights accommodations for 2, including transfers; (c) tickets for 2 to one Broadway show; (d) dinner at Tavern on the Green for 2; (e) admission to Empire State Building for 2 (f) all taxes, sedan driver and gratuities. Package does not include additional nights, additional meals, wine, bar, or incidental expenses. Travel arrangements must be made in cooperation with ANYTOWN TRAVEL which will choose the airline. To make reservations the winner must contact ANYTOWN TRAVEL AT 999-991-1000 at least 30 days prior to anticipated departure. Reservations must include a Saturday night stay and are subject to availability. Reservations must be finalized by September 1, 20XX. Airline tickets may not be upgraded, assigned or transferred and are non-refundable. Trip must be completed prior to November 1, 20XX. Blackout dates are May 26 - June 2, June 3 - July 7, and September 1 -8.
